Registering with the Practice

We can only accept new patients who live within the practice boundary.  Click here to see the practice area.

If you do live in our practice area, you can register by completing a New Patient Registration form PRF1.  You can do this online and we will automatically receive a copy.  Alternatively you can download a copy below and return it to the practice.

Please also complete the Patient Registration Additional Information form.  The Additional Information form contains important information about consent.  All registrations must include BOTH forms.

Please check your answers carefully and ensure you have completed all the questions.  Missing information slows down the registration process as our staff will need to contact you to gain the correct details.  Once completed please bring the forms to the practice or email them to us.

You do not have to provide ID when registering, but it does help us to speed up your application if you can provide a Passport, Birth Certificate and Utility Bill.

Summary Care Record

Please ensure you understand the importance of your Summary Care Record and your ability to choose to opt of sharing this data.  This link will provide more information.  If you decide to opt out, please ensure you complete the opt form and bring it into Reception.

Please complete these forms if you wish to register for Online Patient access.  We will need to see you in person with various ID to verify your identity.  This is due to the confidential information accessible on the online platforms.   The forms below detail acceptable verification ID.

NHS App

What you can do with the NHS App

The NHS App allows you to access a range of NHS services. You can download the NHS App on your phone or tablet. You can also access the same services in a web browser by logging in through the NHS website.  You can:

  • Order repeat prescriptions and nominate a pharmacy where you would like to collect them.  There’s no need to wait to join a telephone queue or wait until the GP surgery opens
  • Book and manage appointments
  • View your GP health record to see information like your allergies and medicines (if your GP has given you access to your detailed medical record, you can also see information like test results)
  • Book and manage coronavirus (COVID-19) vaccinations
  • Get your NHS COVID Pass (there are also other ways to get your COVID Pass)
  • Register your organ donation decision
  • Choose how the NHS uses your data
  • View your NHS number (find out what your NHS number is)
  • Use NHS 111 online to answer questions and get instant advice or medical help near you

You must be aged 13 or over to use the NHS App. You also need to be registered with a GP surgery in England or the Isle of Man. Find out more about who can use the NHS App.
